55.37 - Death Benefits Active Employees

- Life and accidental death and dismemberment insurance benefits available upon death of an employee or dependent is based upon the basic and dependent and supplemental life options elected each year under the Custom Choices for a healthy U and Idaho cafeteria style benefit program.
- The limiting age for dependent children is 19 years or to age 25 if fulltime students.
- Employees and dependents may have rights to life insurance portability in addition to life insurance conversion

A. General. University employees on board appointment may be eligible for certain death benefits such as life insurance, supplemental life insurance, and continued medical/dental coverage for dependents.
B-1. Life Insurance.
i) Basic Group Life Insurance. Basic life insurance equaling one year’s annual salary rounded up to the next $1,000 is provided by the University for all faculty and staff on pay status holding a regular board appointment at 50% time or greater, and working for five consecutive months or longer.
ii) Accidental Death and Dismemberment Insurance. Accidental death and dismemberment insurance provides benefits in addition to group term life insurance for active employees only.
iii) Spouse and Dependent Life Insurance. The basic University life insurance also covers an employee's spouse and legal dependents age 10 days to 23 years. Information regarding benefit amounts may be obtained from Benefit Services at (111) 111-3697.
iv) Spouse and Dependent Conversion Rights. Dependent spouses or children may have rights to covert their coverage after the death of an employee. Conversion of benefits must occur within 30 days of the loss of coverage. More information is available from Benefit Services.
B-2. Supplemental Life Insurance. Several options are available for supplemental life insurance through the University. The primary options are: Principal Financial Group offers a matching amount to the basic life noted above, and PEGLI (Public Employee Group Life Insurance - offered through the Public Employee Retirement System of Idaho). The coverage level for each plan is subject to several factors, including age and salary.
i) Exceptions. Cooperative Extension Service employees who carry federal group life insurance, and Graduate Teaching or Research Assistants, are not eligible for life insurance benefits.
B-3. Medical/Dental Coverage for Dependents of Deceased Active Employees. The University will continue coverage for the surviving dependents of an employee who dies while covered under the active employee health program of the University of Idaho at the time of death. Coverage will be provided via COBRA. COBRA provides up to 36 months of access to coverage. UI will pay the cost of the coverage for up to the first 12 months of COBRA, provided that the surviving dependent(s) qualify or continue to qualify for COBRA. COBRA regulations allow participation only when surviving dependent(s) have access to no other coverage, including another employer plan or Medicare. After the UI pays 12 months, the surviving dependents may continue for an additional 24 months, by paying the full COBRA cost, for as long as they remain COBRA qualified.
i) Notification Required. Continued coverage is not automatic. Benefit Services (111) 111-3697 must be contacted within 30 days of the death.
B-4. Public Employee Retirement System of Idaho (PERSI), Optional Retirement Plan (ORP) or Supplemental Retirements Benefits. Surviving beneficiaries of active employees participating in UI Retirement programs such as PERSI or ORP are eligible for death benefits. B-5. Social Security. Death benefits and/or survivor benefits may be available to dependents of deceased active employees. To determine eligibility and obtain information, contact the Social Security Administration. Contact information:
i) Exception. Graduate Teaching or Research Assistants and those Cooperative Extension Service personnel enrolled in the (federal) Civil Service Retirement System do not participate in the Social Security system.
B-6. Voluntary Benefits. Voluntary benefit options may offer additional benefits or options for continuation of coverage. Beneficiaries should contact these vendors directly. Current contact information for voluntary program vendors affiliated with UI may be found on our website: Benefits Websites.
C. Process. Notification of death should be provided to Benefit Services (111) 111-3697, by the spouse or legal representative of the deceased employee. Benefit Services will assist in completing and filing appropriate forms, and will assist in referring the spouse or legal representative to other agencies.
C-1. Filing Life Insurance Claim. In the event of death on a covered employee or dependent, life insurance claim forms are filed through Benefit Services by the spouse or legal representative of the deceased employee or eligible retiree. In addition to the completed insurance claim forms, a certified Death Certificate is required to be submitted to the insurance company. Insurance claim processing takes approximately 4-6 weeks.
