Accident and Incident Reporting
What we do
EHS has established a web-based accident/incident report to facilitate prompt reporting. All accidents, incidents and near misses should be reported as soon as possible to facilitate the mitigation of the hazardous situation as quickly as we can. Accidents where the employee seeks medical attention need to be submitted though EHS for processing by the State Insurance Fund, see also Workers Compensation.
What you can do
Every individual on campus is responsible to say something if they see something that is unsafe. The prompt reporting of injuries or illnesses related to university employment or activities is essential to ensure that the person(s) involved have received the proper medical care, the situation causing the injury or illness has been identified, and the appropriate documentation has been completed.
