Animal Care and Use Forms

Institutional Animal Care and Use Committee (IACUC) is using the electronic system for protocol submissions, amendments, adverse events and protocol deviations. Instructions for reporting adverse events and protocol deviations can be found by visiting the Reporting Adverse Events and Protocol Deviations page (NetID login required) on the Animal Care and Use (IACUC) website.

When submitting a renewal protocol, it is the principal investigator’s responsibility to ensure all previously approved procedures that are ongoing are included on the renewal protocol form.

All IACUC Submissions must be turned in by the close of business on the date of submission deadline in order to be considered at that month's meeting.

A Veterinary Drug Order Form* must be used to order controlled drugs and other veterinary prescription drugs and supplies. Submit the completed form to the Campus Veterinarian at [email protected].
